Are trained to provide exceptional NHTD and TBI services designed to
meet each client’s needs, choices, strength and goals with the
following conditions:
Dementia, Alzheimer, Traumatic brain injuries, Parkinson’s, Cognitive
issues (Challenges with thinking), Behavioral issues (Challenges with
acting), Stroke, Spinal cord injury, May be Elderly and frail clients.
Our caregivers are consistently trained, to be patient, treat clients
with respect, calm and controlled behavior, expect the unexpected and
be consistent.
Must be 65 or older or for adults 18 and
older with a traumatic brain injury
Participant must be eligible for a nursing
home level of care.
Be authorized to receive Medicaid community
based long term care
Participant must be capable of living in the
community with needed assistance from available informal supports,
non- Medicaid supports and/ or Medicaid state plan services and need
one or more waiver services such as home level of care.
